Abstract

Background: Postoperative urinary retention (POUR) is an important but incompletely characterized complication of spinal anesthesia for lower limb orthopedic surgery. Although intrathecal adjuvants enhance the quality and duration of bupivacaine anesthesia, improved analgesia does not necessarily translate into earlier recovery of micturition. This narrative review integrates lower urinary tract neurophysiology, the pharmacology of bupivacaine and its two commonly investigated adjuvants—fentanyl and dexmedetomidine
