Background: Acute kidney injury (AKI) is common in critical illness and remains difficult to detect before clinically apparent loss of filtration. Magnesium is central to cellular energy handling, vascular tone, inflammatory signaling, and tubular transport. Hypomagnesemia is frequent in intensive care and has been associated with AKI, but its clinical interpretation is complicated by illness severity, medications, renal losses, and the limitations of total serum magnesium measurement.
