Background: Ultrasound-guided posterior quadratus lumborum block (QLB) is increasingly incorporated into multimodal analgesic pathways for abdominal and pelvic surgery. Although bupivacaine provides effective regional analgesia, the duration of a single-injection block may not cover the entire period of clinically important postoperative pain. Dexamethasone and magnesium sulfate have therefore been investigated as adjuvants capable of extending analgesia and reducing opioid requirements.
